Ordering test requisition forms

To order test requisition forms for Zoetis Reference Laboratories (ZRL) tests, follow these steps.

Note: Requisition forms come in packs of 25. Enter the number of packs needed for each form.

  1. Choose Support from the side menu.

  2. Click Form Order in the ZRL Customer Assistance section at the bottom of the page.

  3. Enter desired information. Your name and email will be entered for you from your user account information. All fields marked with * are required.

  4. Click Submit or Cancel to exit without submitting.
